Submission Requirements

In order to provide the best products and services, workers’ compensation applications must include:

• Minimum premium of $25,000 for a stand-alone program; no minimum with a package.
• Accord forms.
• Copy of most recent NCCI mod worksheet.
• 3-5 years currently valued, hard copy loss history with summary attached.
