Submission Requirements

In order to provide the best products and services, construction applications must include:

• Accord forms.
• Completed contractors questionnaire application.
• Schedule of properties to include full COPE information and property values.
• Drivers list and auto schedule to include GVW and cost new.
• Employee Practice Liability Insurance application (if coverage is required).
• AIG Pollution Legal Liability application (if coverage is required).
• AIG Environmental application addendum mold questionnaire (if coverage is required).
• 3-5 year currently valued, hard copy loss history with summary attached.
